KDIGO Controversies Conference Blood Pressure in Dialysis Patients
Kidney Disease: Improving Global Outcomes (KDIGO®) hosted a two-day controversies conference on “Blood Pressure in Chronic Kidney Disease Stage 5/ Dialysis Patients” in New York City. Fifty nephrologists, cardiologists, and researchers participated in the conference, which began with plenary session presentations reviewing the newest research on the evaluation, natural history, pathophysiology, and clinical management of blood pressure in patients receiving renal replacement therapy. The presentations were followed by focused discussion groups and a consensus-building process resulting in recommendations that will be published in a position statement.
A primary goal of the conference was to review the quality of the available evidence and make a recommendation to KDIGO on the validity and usefulness of developing a clinical practice guideline in this area. The group was also asked to determine if consensus could be reached on specific clinical recommendations that could be included in a position statement and, maybe most importantly, were challenged to outline and prioritize specific research projects that would be most valuable in expanding our knowledge in how to manage blood pressure in CKD Stage 5D.
“What will be the outcome of our gathering?,” asked the KDIGO co-chair and conference leader, Bertram Kasiske, MD during his opening speech. “Will we send a message to clinicians to improve patient outcomes with recommendations, or will we call for more research, or will we decide it is time to update or begin working on a new clinical practice guideline?”
The sessions were moderated by conference co-chairs, Nathan Levin, MD of the Renal Research Institute in New York City and Gérard London, MD of Centre Hospitalier Manhes in Fleury-Mérogis, France.

“We are impressed with the focused and productive dialogue that this distinguished group of international experts engaged in during the conference,” said Kai-Uwe Eckardt, KDIGO Co-Chair. “The recommendations that evolved from the discussions will be very valuable for the nephrology community and for the KDIGO Board to consider in planning for future clinical practice guidelines.
Look for the recommendations from the conference to be published in Kidney International in the next year.
The next KDIGO controversies conference will be held in the fall of 2009 to address the topic of “Chronic Kidney Disease: Definition, Classification and Prognosis.”

As a first taster of KDIGO I have been impressed by the organisation, the organisation (the former the body, the latter the "on the ground" component) and above all the spirit in which this meeting was conducted. The fact that it was in one of the few truly great global cities was another special bonus.

I would like to thank KDIGO for undertaking the efforts to organize an international controversies meeting on the much debated issue of blood pressure treatment in CKD5. It is indeed a timely undertaking!
