• Systemic anticoagulation should be avoided. Preferred alternatives are no anticoagulation with regular flushing of the circuit or regional citrate anticoagulation (Level A)
• Regional anticoagulation with heparin and protamin neutralization should be avoided because of risk of bleeding after dialysis (Level A)
